Admission for continuous or day hospitalisation at Panciu Town Hospital

2. General Information

Admission is the way in which a patient with a particular condition receives medical care in an inpatient facility.

Depending on the type of admission, these can be classified as:

• Admissions – medical and surgical emergencies – situations in which the patient's life is endangered. Account is taken of the fact that each ward/unit is required to keep a number of free beds, between 5-10% of the total number of beds in the ward/unit.
• Admission made on the recommendation of the family doctor, the specialist doctor in outpatient healthcare facilities regardless of their form of organisation, doctors in medical and social care units, or doctors in dialysis centres under contract with CNAS.
• Admission by inter-hospital transfer
• Admission on request

Admission conditions generally applicable to all types of admission except medical and surgical emergencies

Documents required for admission:

• The referral note from the family doctor/specialist doctor or the medical letter issued by doctors in the hospital's integrated outpatient department who are under contract with CJAS VN, or a medical letter issued on discharge with an indication to return for admission.
• Identity card / passport documents, as applicable
• Birth certificate for children under 18
• Employment / student / pupil certificate or pension slip
• HEALTH CARD
• The national health card will be mandatory on admission to hospital in order to access hospital medical services under continuous hospitalisation, except in the following situations:
• On admission to hospital, if the admission criterion is a medical or surgical emergency, no referral (admission) note is required
• When transferring patients to other healthcare facilities
• Upon the patient's death

Admission scheduling:

• Depending on the specifics of each ward/unit and on bed availability, admission may be scheduled. 
• For scheduled admission at Panciu Town Hospital, the attending doctor:

• Requests the referral note from the family doctor/specialist doctor or the medical letter issued by doctors in the hospital's integrated outpatient department who are under contract with CJAS VN, or a medical letter issued with an indication to return for admission;
• Informs the patient about the diagnosis and the procedures to be performed
• Informs the patient about the hospital's capacity to cover the costs of medical treatment on the basis of the waiting list

Appointments are made:

• By phone, through the hospital switchboard or by calling directly the numbers of the external wards/units, the patient agrees the admission date directly with the doctor.
• Directly – the patient sees the doctor and is scheduled for admission on a later date, by mutual agreement.
  
  

The priority criteria for access to the services offered in the case of waiting lists that do not constitute medical emergencies are:

• The patient's clinical condition
• Distance between home and hospital
  
  

On admission the patient must present:

• Passport documents - identity card, identity booklet, birth certificate
• Health card / EHIC certificate
• Proof of insured status – triage staff can check the patient's insured status by accessing the CNAS insured persons database.
• The admission note signed and issued by the outpatient doctor/family doctor and endorsed by the attending doctor of the ward to which the patient is to be admitted.
  
  

Admission procedure:

• The patient to be admitted must report to the triage area (of each ward) from 7.00 a.m. with the following documents:

• An admission referral note from the family doctor/specialist doctor under contract with CJAS VN, or a medical letter issued by the specialist doctors in the hospital's integrated outpatient department who are under contract with CJAS VN, or a medical letter issued on discharge with an indication to return for admission.
• Proof of insured status (employment certificate, pension slip, voluntary insurance)
• Copy of identity card
• Health card
• The nurse with registrar/data operator duties checks the patient's insured status in the CNAS database – Insured status check
• After checking insured status (insured/uninsured, personal identification number not present in SIUI), the admission is entered in the application used by the healthcare facility. The general clinical observation record (FOCG) is assigned a number; the medical registrar/nurse with registrar/data operator duties will note on the FOCG (in paper form) the record number (allocated by the application) and the date and time of admission, and will attach the proof of insured status, the admission note issued by the family doctor/specialist doctor or the medical letter issued by doctors in the hospital's integrated outpatient department who are under contract with CJAS VN, or a medical letter issued on discharge with an indication to return for admission, and the patient's informed consent. The patient's admission will be signed with the national health insurance card in the IW CEAS application used by the healthcare facility.

Admission of patients through the Emergency Room

• The emergency department is open to all patients, insured or uninsured, who request emergency medical care following the onset of acute complaints or against the background of chronic conditions.
• On the patients' arrival, after triage, patients in the Emergency Room are examined by the doctors on duty in these units, who will decide on the necessary investigations and on the specialist doctors to be called in to provide a specialist consultation, if they consider it necessary.

Admission of a minor child

On admission of a minor patient, the accompanying person must present the following documents:

• The referral note for admission from the family doctor/specialist doctor under contract with CJAS VN, or the medical letter issued by doctors in the hospital's integrated outpatient department who are under contract with CJAS VN, or a medical letter issued on discharge with an indication to return for admission.
• Copy of the birth certificate
• School enrolment certificate
• Children benefit from compulsory insurance, subject to being registered in SIUI (the Unique Integrated IT System), the CNAS IT platform where all insured persons are registered. Minor children may be admitted with an accompanying person.
• For a minor patient presenting at the Emergency Room, no referral (admission) note is required

Admission of insured/uninsured patients

Under the health insurance law in force, all patients resident in Romania are entitled to free emergency medical care.

Medical services reimbursed by CNAS or the OPSNAJ House throughout the hospitalisation, regardless of whether patients are insured, and recorded in SIUI (the Unique Integrated IT System).

Insured patients receive the maximum package of medical services.

Uninsured patients are admitted through the Emergency Room and receive emergency medical services, the attending doctor being obliged to assess the patient's medical situation and to discharge them once emergency medical services are no longer justified. For continuous hospitalisation, at the patient's request, admission may continue under continuous hospitalisation with the patient bearing the costs of the hospital medical services for the period in which the services no longer constitute an emergency.
